Online Enrollment
Online enrollment is the fastest and easiest way to enroll. Before you enroll, have the following information available:
- Your investment allocations, which you select from your investment choices
- Your Social Security number
- Your beneficiary's Social Security number, birth date and address
Next, choose the plan(s) in which you want to enroll*:
Enroll By Mail
To enroll, follow these instructions:
- Download the enrollment materials below
- Complete the fields as indicated
- Sign the forms as needed
- Submit the forms* to:
