You must complete the following forms in order to enroll: - A Salary Reduction Agreement allows automated, pre-tax contributions from your paycheck directly to your specified plans. Click the link(s) below to download and complete the SRA for your chosen plan:
- If you want to allocate some or all of your contributions to mutual funds (including Lifecycle Funds) offered in your Supplemental plan, you also must sign an Individual Custodial Agreement, which is included with the enrollment application. You can download a copy of the Individual Custodial Agreement for your records.
Enroll Online Online enrollment is the fastest and easiest way to enroll. Before you enroll, have the following information available: - Your investment allocations, which you select from your investment choices
- Your Social Security number
- Your beneficiary's Social Security number, birth date and address

In addition, the Salary Reduction Agreement linked above is an agreement between you and Marquette University to reduce your taxable income by a dollar amount or percentage of salary each pay period. You must submit a Salary Reduction Agreement to Marquette's Department of Human Resources, Straz Tower, Room 185 along with your enrollment materials. (If you enrolled using the TIAA-CREF online enrollment facility, please submit the Salary Reduction Agreement and notify a representative from Marquette's Human Resources department that you have completed the online enrollment.)
