You must complete the following forms in order to enroll:

• A Salary Reduction Agreement allows automated, pre-tax contributions from your paycheck directly to your specified plans. Click the link(s) below to download and complete the SRA for your chosen plan:

Online enrollment is the fastest and easiest way to enroll. Before you enroll, have the following information available:

• Your investment allocations, which you select from your investment choices
• Your Social Security number
• Your beneficiary's Social Security number, birth date and address

• Oklahoma State University Alternate Retirement Plan (ARP)
• Oklahoma Board of Regents for the A&M Colleges TDA Plan
• Oklahoma State University 457(b) Deferred Compensation Plan
• Oklahoma State University 403(b) Retirement Plan Prior to 7-1-89
