- If you want to allocate some or all of your contributions to mutual funds (including Lifecycle Funds) offered in your Supplemental plan, you also must sign an Individual Custodial Agreement, which is included with the enrollment application. You can download a copy of the Individual Custodial Agreement for your records.

When electing employee contributions, you must complete a Salary Reduction Agreement. Please go to your Human Resources office to complete and return the agreement along with your enrollment form.
